The organ clock: why your liver works harder at night than you do

Ever heard of the organ clock? No? You're definitely not alone. The so-called organ clock comes from Traditional Chinese Medicine (TCM): each of the twelve organs is assigned two hours in which particularly much energy, the Qi, flows through it. Twelve hours later, it rests. Quite fairly organised, really.
The idea is a few thousand years old and comes from a time when the body was observed without being seen into. That is exactly what makes it interesting: note down over generations when people wake and when they feel unwell, and patterns emerge without a laboratory. Whether the explanation is right is a different question from whether the observation is.
It gets especially interesting at night. Between 9 and 11 pm the “triple burner” takes command, less an organ, more a system connecting all energy circuits. Translated: the body winds down, pulse and blood pressure sink. Time to relax.
From 11 pm to 1 am the gallbladder is active, processing rich food and detoxifying. Which is exactly why heavy late-night meals are about as helpful as an espresso before bed. Between 1 and 3 am the liver runs at full speed, and yes: alcohol gives it plenty of extra work. According to TCM, people with liver trouble often wake precisely in this window.
From 3 to 5 am the lungs take centre stage, maybe grandma wasn't entirely wrong about sleeping with the window open. Between 5 and 7 am the large intestine reports for duty (a glass of warm water in the morning helps), and from 7 to 9 am the stomach would, according to TCM, much prefer a warm breakfast to hastily gulped coffee to go.
What does this mean for sleep? Evening relaxation, light meals and less alcohol support the body's night shift. Alcohol between 11 pm and 3 am is considered an absolute sleep killer, gallbladder and liver get involuntary overtime.
And what does modern sleep research say? Surprisingly much overlap, on entirely different grounds. The body does have an internal clock, but it sits in the brain and is set mainly by light, not by Qi travelling from organ to organ. What both models share is the basic claim: the body does not pause at night, it works to a plan.
And yes: the organ clock is no exact timetable; every body works differently. But the idea that our body works diligently at night while we sleep is somehow reassuring. Or at least while we try to sleep.
For waking at three there is even a fairly solid explanation. Towards morning cortisol rises again, blood sugar is at its lowest, and sleep naturally becomes lighter. Waking in that phase is therefore nothing unusual. Whether you fall asleep again or lie there thinking about tomorrow decides more about the night than the waking itself.
And if despite all this you regularly wake with pain that fades over the morning, the cause is probably neither the liver nor the hour, but simply what you are lying on. That is the least romantic part of the night shift, and often the decisive one.
On alcohol, TCM and the sleep laboratory agree anyway, only in different words. What is measurable: alcohol shortens the time to fall asleep and then takes apart the second half of the night, with less dream sleep and more waking. That the liver is particularly busy in exactly that window fits both accounts.
What is practical about the organ clock is therefore less the assignment than the reminder that the evening prepares the night. Eat lightly, stop drinking early enough, turn the light down, and do not spend the last hour with your head in work mode. Whether you call that Qi or chronobiology, the result in the morning is the same.
A word of caution, because the internet often says otherwise: no diagnosis can be drawn from the hour you wake. Waking regularly at three does not automatically mean a liver problem, any more than a dry throat at four means a lung problem. If complaints persist, they belong with a doctor and not in a table.
