Which mattress for back pain? What actually matters

Back pain has many possible causes, and no mattress can replace diagnosis or treatment. Yet the surface beneath you affects how the body rests for eight hours. An unsuitable mattress can aggravate existing discomfort, while a well-matched one can reduce pressure and allow a more relaxed sleeping position.
Is a hard mattress better for the back?
Not automatically. On a mattress that is too hard, shoulder and pelvis cannot settle sufficiently. The spine compensates, muscles remain tense and pressure points develop. A mattress that is too soft lets the pelvis drop. The aim is not maximum hardness but a balance of give and stable support.
Side, back and front sleepers
On the side, the shoulder must sink farther than the waist so the spine does not bend laterally. On the back, the pelvis should be received without collapsing while the lumbar area keeps contact and support. Front sleepers often require a more stable centre. The pillow is part of the system too: if it is too high or low, neck tension can remain even when the mattress is right.
Pocket springs and precise support
Individually working pocket springs respond where weight is applied. The shoulder can yield without the entire surface dropping. Multiple spring layers and different spring tensions allow finer adjustment. Natural materials such as wool, cotton and horsehair also manage the sleeping climate so heat and moisture interfere less with rest.
How to test a mattress properly
Do not simply lie on your back for a minute. Use your normal sleeping position, stay for at least ten minutes and notice whether you are unconsciously holding tension. In side sleeping, another person can check whether shoulder, spine and pelvis form a calm line. Then test one slightly softer and one slightly firmer option so the contrast becomes meaningful.

Severe, new or persistent pain, numbness, loss of strength or radiating symptoms require medical assessment. Bed advice can assess comfort and support, not diagnose the cause of pain. It is particularly useful when discomfort is worse in the morning, the same pressure points recur, or the existing mattress visibly sags.
